Online Booking Form
Follow the prompts in the online booking form below to complete your booking. Please make sure that you have checked the following two settings before booking:
1) Make sure that you select the correct clinic from the dropdown menu. There are four options Paradigm Urology Bendigo, Every Heart Clinic Woodend, The Jewel Melbourne and Martin Gladman Online.
2) Once you have selected your clinic and the service you would like to book in for you will move to Step 2 in the process where you select the time of your session. Please make sure that your timezone is set correctly in the dropdown menu.
